The forms below are the current forms for the 2008 football and cheer season. We are giving parents the opportunity to fill out forms before the registration dates. Each line on all the forms MUST be filled in. Football players must have the most accurate weight to assist in team placement. Please make sure phone numbers are the same on all forms.

Donor / Sponsor Forms

This is an excellent opportunity for parents/businesses to show your support for Merrillville Pop Warner!! Parents can send a personalized message to their child in our MPW yearbook! Businesses can use this as a tool to not only support our community but also promote your business! There are hundreds of parents that will be traveling to Merrillville this upcoming 2008 season from surrounding communities. Lets show them a BIG welcome and show our PIRATE PRIDE!!!

2008 Physical Forms

The physical form is two pages.

FIRST MED in Merrillville @ 751 E. 81st Place (Across from the Raddison) is offering physicals for $20.00 for MPW Players. Physicals MUST be done on our forms.
